One of the licensing requirements approved is compliance with the procedure for their storage in accordance with the legal norms of Decree of the Government of the Russian Federation of December 31, 2009 N 1148.

This resolution states that stocks of narcotic drugs and psychotropic substances, as medicines intended for medical use, are determined by legal entities on the basis of standards established by the Ministry of Health of the Russian Federation for calculating the need for these medicines.

These standards were previously regulated.

From January 1, 2017, standards for calculating the need for these medicines are established.

How are standards calculated?

From this order data is taken for a medicinal product in relation to the type that turns out to be and multiplied by the average data from the report of a medical organization on the use of narcotic drugs and psychotropic substances for the last three previous years.

If the reserves of narcotic drugs and psychotropic substances used as medicines obtained by this calculation method are not enough due to the nature of the activity, then by decision, for example, issued by order, these reserves can be increased, but no more than 1.5 times.

Calculations must be kept by the medical organization and provided upon request.

And of course, there should be no discrepancy between the actual availability of reserves and the calculated data. Otherwise, there will be a violation of licensing requirements and prosecution.

Thus, before the new year, these calculations must be completed and by January 1, 2017, stocks of narcotic drugs and psychotropic substances intended for medical use must meet the requirements.

5. If a congenital and (or) hereditary disease is suspected or detected in a patient that does not require hospital treatment, a local therapist, a local pediatrician, a general practitioner (family doctor), medical workers with secondary medical education from medical organizations if there are medical indications, the patient is referred (if there are medical indications, his family members) for a consultation with a geneticist at the medical genetic consultation center (center).

ORGANIZATION OF MEDICAL-GENETIC ACTIVITIES

CONSULTATIONS (CENTER)

1. These Rules establish the procedure for organizing the activities of medical genetic consultation (center) of organizations providing medical care (hereinafter referred to as medical organizations) to patients with diseases.

2. Medical genetic consultation (center) (hereinafter referred to as the Consultation) is an independent medical organization or a structural unit of a medical organization created to provide preventive, advisory, diagnostic and therapeutic care to patients with congenital and (or) hereditary diseases, including their members families (hereinafter referred to as patients).

3. A specialist is appointed to the position of head of the Consultation who meets the requirements of the Qualification requirements for specialists with higher and postgraduate medical and pharmaceutical education in the field of healthcare, approved by order of the Ministry of Health and Social Development of the Russian Federation dated July 7, 2009 N 415n (registered by the Ministry of Justice Russian Federation on July 9, 2009, registration N 14292), as amended by order of the Ministry of Health and Social Development of the Russian Federation dated December 26, 2011 N 1644n (registered by the Ministry of Justice of the Russian Federation on April 18, 2012, registration N 23879), in the specialties: “genetics”, “therapy”, “obstetrics and gynecology”, “pediatrics”, “health care organization and public health”.

The head of the Consultation is appointed to the position by the founder of the medical organization or the head of the medical organization if the Consultation is created as a structural unit of the medical organization.

4. The structure and staffing of the Consultation is established by the founder of the medical organization or the head of the medical organization in cases where the Consultation is created as a structural unit, based on the volume of diagnostic and treatment work carried out, the number of the population served and the number of births per year, taking into account the recommended staffing standards provided for in the appendix N 2

5. The equipment of the Consultation is carried out in accordance with the standard of equipment of the Consultation provided for in Appendix No. 3 to the Procedure for providing medical care to patients with hereditary and (or) congenital diseases, approved by this order.

Mass screening laboratory;

Selective screening laboratory;

Laboratory of molecular genetic diagnostics;

Department of prenatal diagnostics, including an office of an obstetrician-gynecologist, a manipulation room (operating room), and an ultrasound diagnostic room;

Procedural;

A room for receiving samples of blood, urine and other biological material for laboratory diagnosis of hereditary and (or) congenital diseases.

Administrative and economic department;

Information and analytical department, including a registry, an organizational and methodological office, and a medical statistics office.

8. The consultation carries out the following functions:

Providing consultative, therapeutic and diagnostic assistance to patients and members of their families;

Organization and provision of neonatal screening in order to identify hereditary diseases;

Conducting selective screening;

Organizing the collection of blood samples from pregnant women for prenatal biochemical screening;

Conducting prenatal biochemical screening of pregnant women in the first trimester (1st stage) for the purpose of prenatal (antenatal) diagnosis of child development disorders;

Conducting the 2nd (confirming) stage of a comprehensive examination of pregnant women for the purpose of prenatal (antenatal) diagnosis of child development disorders;

Conducting prenatal cytogenetic, molecular cytogenetic and molecular genetic diagnostics in women who have an increased risk of chromosomal diseases in the unborn child based on the results of combined prenatal screening in the first trimester of pregnancy and ultrasound in the second trimester of pregnancy;

Carrying out prenatal diagnosis of congenital and (or) hereditary diseases in burdened families;

Participation in external expert assessment of the quality of laboratory tests performed during mass examination of newborn children for hereditary diseases;

Organization and provision of methodological and advisory assistance to local pediatricians, local general practitioners, obstetricians-gynecologists, general practitioners (family doctors) in order to identify patients at risk with hereditary and (or) congenital diseases, as well as patients with initial manifestations hereditary and (or) congenital diseases;

If there are medical indications, patients are referred for consultation to medical specialists in the specialties provided for by the Nomenclature of Specialties of Specialists with Higher and Postgraduate Medical and Pharmaceutical Education in the Healthcare Sphere of the Russian Federation, approved by Order of the Ministry of Health and Social Development of the Russian Federation dated April 23, 2009 N 210n;

If there are medical indications, refer patients for medical care in an inpatient or day hospital setting;

Carrying out clinical observation of patients assigned to the territory served by the Consultation;

Participation in the analysis of the main medical and statistical indicators of morbidity, disability and mortality of patients assigned to the territory served by the Consultation;

Organization and conduct of sanitary and educational work among the population on the prevention of hereditary and (or) congenital diseases, the formation of the principles of a healthy lifestyle;

Participation in the development and organization of implementation of individual programs for rehabilitation and treatment of patients;

Participation in the preparation of medical documents of patients to refer them for medical and social examination;

Participation in the analysis of the main medical and statistical indicators of morbidity, disability and mortality of patients;

Development and implementation of new effective methods of prevention, diagnosis, treatment and rehabilitation of patients;

Maintaining accounting and reporting documentation, submitting reports on the activities of the Consultation in the prescribed manner.

9. To ensure its activities, the Consultation uses the capabilities of all treatment, diagnostic and auxiliary units of the medical organization within which the Consultation was created.

10. The consultation can be used as a clinical base for educational institutions of secondary, higher and additional vocational education, as well as scientific organizations.
